Besides purchasing insurance online, the staff seemed to push additional coverage insurance, since you'll eventually need to buy insurance anyway when traveling to southern Italy. This is a matter of personal choice! The important point is that the insurance cost is about the same as the car rental fee, which is quite expensive, but it's good to have extra protection for your safety.
